forms for pet acupuncture
The following forms are designed to help us work together more easily.
Pet Owners are asked to complete the following forms before a session can be scheduled.
New Patient Health History Form: filled out by the owner and sent to me via email 4pawsacupuncture@gmail.com
Veterinarian Referral Form (mandatory before treatment) : Completed by your veterinarian and sent to me via email or faxed to Our Family Veterinary Services: 978-535-8205.
After receiving both forms, I will schedule a phone consultation to discuss your pet's symptoms, veterinary diagnosis, any painful areas, diet information and herbal supplements given or other issues. This information helps determine which therapies, herbs and remedies work best to relive your pet's pain and symptoms. It also saves time during the first session so I can concentrate soley on treating your pet. Thank you for taking the time to fill out the forms completely ahead of time.
